An issue is an unplanned event that has happened and requires management action. An issue might be a concern, query, suggestion, request for change or anything else to do with the project.
It is important to manage issues – if issues remain unresolved they may result in changes to the Project Plan, timescales, cost, scope, quality, risks or benefits realisation.
Issues should be recorded in an Issue Log which is frequently reviewed by the Project Manager and project team. Significant issues should be communicated to the Project Sponsor and the Project Board as part of the regular progress reporting process.
The Issue Log is often part of the same document as the Risk Register. A risk may turn into an issue if the conditions which give rise to the risk occur, and the mitigating actions which were identified as part of the risk management approach may also be appropriate to address issues.
Depending on the size or complexity of your project you may decide that a formal Issue Log is not required. However, if a formal log is not maintained it is still important to be aware of any issues so that corrective action can be taken.
